Intracanal Medicaments and Discolouration Potential - Facts, Fiction, Mechanism & Minimising Strategies.

Dr Michael Khoury

BDSc, DCD(Endo) UWA, MRACDS (Endo)

Although dental aesthetics are influenced by several factors, tooth discolouration is considered the most significant factor behind the patient’s dissatisfaction with their dental appearance. There are several endodontic materials that can put teeth at risk of discolouration -these include pulp capping materials, root canal irrigants, medicaments, root filling materials and cements. Antibiotic-corticosteroid based medicaments are useful aids during endodontic treatment, especially when there is inflammation of the pulp and/or peri-radicular tissues – such as pulpitis, internal and external inflammatory root resorption, apical periodontitis and/or neuropathic pain. The therapeutic effects of these medicaments are mainly a result of the corticosteroid component. The antibiotic component also poses several advantages. When this antibiotic is a tetracycline, the risk of tooth discolouration may increase. However, the degree of discolouration has been controversial with various proponents of commercial medicament pastes claiming superiority of their paste over others. In this lecture, the latest research will be discussed regarding the discolouration potential together with the mechanisms and minimising strategies and techniques to consider using if discolouration occurs.

Dr. Michael Khoury finished his BDSc in 2002 from the University of Jordan. Since then, he has been passionate about delivering quality and evidence based dental treatment to all patients. He gathered his extensive experience by working at different places overseas and in Australia including the Army, the United Nations, UWA and private practice. He worked and taught at UWA School of Dentistry for the last 12 years in all aspects of clinical and preclinical dentistry. He focuses on long term relationship with patients as it is the key to successful prevention of dental disease and improving oral health. In 2020 he completed his DCD in Endodontics at UWA. He works now as a full time Endodontist at WA Endodontics. In 2006 he was awarded the United Nations Service Medal and the Jordanian Peace-Keeping Medal. He is a member of the ADA and has membership with the RACDS in Endodontics. He is currently the ASEWA secretary.
